The ability to sign up and request care will be available soon!

 

All veterans will be asked to fill out a short form containing basic background information about you, your military service and what your biggest concerns are currently.

The information gathered from this form will help us match you with a Dentist/provider nearest to your location who is participating in our Vet Dental Network program, and has agreed to provide dental services a certain amount of veterans each month or year.

​

The form you submit will also put your name on our e-mail list (which goes nowhere but our files, because otherwise that's just rude) so when we have events or news that may concern you or your area, we can let you know!
